v0.1.27 - one tombstone per player, tombstones on dedicated servers, a real mausoleum

• every player gets exactly one tombstone per world, matched by player name: no more two stones for one player
• tombstones now appear on dedicated servers (before, they only appeared while a host stood inside the crypt)
• the mausoleum places itself once per world, including worlds made before it was installed: further out when the middle is taken, never on a player's build, and in a zone of its own on one-zone-at-a-time worlds
• the entrance is a stone temple with a slate gable roof, on levelled ground with fallen masonry around it
• rebuilt crypt rooms: flagstone floors with broken slabs, marble trim, coffered ceilings, a sarcophagus hall, rubble and green torches
• memorial cards now count deaths (needs FiresNPCs 0.0.460 or newer)
• no more periodic hitch from tombstone upkeep on large worlds: the crypt and its stones are found in small slices each frame instead of a whole-world search every few seconds
