Bugfix.

Adjusted the Healer set effect cooldown behavior. Moving Healer armor pieces between chests and item stands no longer triggers the cooldown. The cooldown is now only applied when removing a Healer armor piece while the full set effect is active.
Added dynamic tooltip support for the Healer set re-equip cooldown time.
Reduced elemental buffs from Codex.
Several item icon and set effect icon adjustments.
